We require that all employees complete a Covid-19 Symptom Attestation Google form prior to reporting to any District facility. The form only takes a few seconds to complete and requires employees to indicate if they have been free of COVID-19 symptoms. Employees will be cleared to report to District facilities based on their responses.
The form must be completed on a daily basis, no more than two-hours prior to reporting to a district facility. Employees may not report to a district facility until the form is completed and they have been cleared to report to the site. Employees who are working remotely do not need to complete the form.
