Sports Physicals and Athletic Packet information for 2013-14

STEP ONE - Athletic Packet and Consent Forms
  (Please remember to complete all 6 steps in the process.)

1. Click this link ONLY TO READ the District Athletic Packet.
It is imperative that you read through the document and are aware of District Policies and Requirements.
 
2. Access the Online Heritage Athletic Packet and fill in the required information online.
By clicking this link you acknowledge that you have read and are aware of the District Policies and Requirements that are stated in the District Athletic Pack link in #1. 
     -Use the highlight fields button on the upper right of the form to see which fields are required.
     -Please make certain you are entering information in the appropriate box.
      
3. Click the submit button in the upper right hand corner. Follow the instructions to e-mail an electronic copy to our office.  (If you would like, you may save a copy of the forms to your computer for your records.)
     -Make certain to submit your forms before printing.  When you submit there is a check to make certain all information was entered.
 
4. Print the completed Consent Forms and make certain that the parent/guardian and athlete both sign the forms in all of the required locations.
 
5. Take the signed forms to your doctor for a physical examination.  Please have your doctor stamp and sign the form.
 
6. Turn in the completed and signed forms to the Athletic Office.

 in the same shopping center as Blockbuster Video and Menifee Tires.
 
Dr. Sheridan's office is very good at getting our athletes in quickly however,
please call ahead to make sure they are open and available.
 

Mandatory Medical Insurance info and Plan for Purchase  (if needed)


All students must have medical insurance in order to participate in sports. If you do not currently have medical insurance you can purchase an accident plan that will cover your student in the event of injury during the school year. Transfer Student CIF Eligibility information and Form

 

If you are a new student to Heritage HS and played sports at a high school last school year, you will need to download,
fill out and bring the form below to the athletic office ASAP.

NEW CIF TRANSFER FORM

 

 CIF has changed the rules for 2012-13. Juniors and Seniors who transfer to a new school without a valid change of address will be required to sit out the first 30 days of competition in each sport they participated in at their former school. 

Students who enroll on the first day of the school year will gain eligibility on the following dates:

               Fall  October 1, 2012, Winter  January 1, 2013,  Spring April 1, 2013

  • Students who move from outside of the Heritage HS boundary area will gain immediate eligibility provided that the entire family moved and the student is living with their legal guardian.

  • Students must sit out for every sport they played at their former school (any level)

  • Students will be allowed to tryout and practice during the sit out period, they will only be held out of games.

  • Transfer students will be allowed to play JV but must play JV the entire season if they choose

  • Sophomores who transfer and are enrolled at their new school when school begins will still be granted immediate eligibility (this year only).

Below you will find the Heritage HS Travel Release form and the Travel Release Policy and Guidelines Form.  Please take a look at the policy form and understand that all of our policies have been put into place in order to protect your child.  Please turn in completed Travel Release forms atleast 24 hours  before the contest in question and remember that coaches DO NOT carry travel release forms.
